##美國醫師執照考試第一階段 (USMLE Step 1):<br />
###目的: 評量考生是否瞭解、並有能力去應用執行醫療之重要的基本科學概<br />
念,並特別強調健康、疾病與治療模式之基本的原理與機制。<br />
###方式: 大約 308 題多項選擇題 (MCQ),區分為七個段考44個項目,每<br />
一個段考時間 60分鐘。要考一整天 – 八個小時。<br />
###有關於第一階段測驗 (Step 1) 的報名、申請書、考題練習、與最新發展<br />

##美國醫師執照考試第一階段測驗基本科學原理 (USMLE Step 1 tests the Basic Science Principles):<br />
###考試科目涵蓋下列傳統的基礎學科的內容:<br />
anatomy, behavioral sciences, biochemistry, biostatistics and epidemiology, microbiology, pathology, pharmacology, physiology.<br />
###同時包括下列跨學科領域的內容:<br />
genetics, aging, immunology, nutrition, molecular and cell biology.<br />
###第一階段考試是為了確保醫學生,不僅於現在精通提供安全與適當醫療的<br />
基礎的科學,而且專長於為了經由終身學習,以維持其能力,所需要的科學原理。<br />

A 45-year-old man comes to the physician because of right shoulder pain that began after he chopped wood 2 days ago. Examination of the Right upper extremity shows no obvious bone deformities or point tenderness. <br />
The pain is reproduced when the patient is asked to externally rotate the shoulder against resistance; there is no weakness.<br />
In addition to the teres minor, inflammation of which of the following tendons is most likely in this patient? __________. (A). Infraspinatus. (B). Pectoralis. (C). Subscapularis. (D). Supraspinatus. (E). Trapezius.<br />

#台灣第一階段醫師國家考試時,基礎學科在國考命題面臨的困境如下:<br />
##「教、考、用」三者間的配合是否緊密?<br />
##試題是否反映應考人須擁有基本科學概念,並特別強調健康、<br />
##疾病與治療模式之基本的原理與機制 (USMLE Step 1) 。<br />
##試題是否反映臨床對應考人須擁有批判思維與知識整合能力之需?<br />
##考照及格率偏低,是否會導致臨床出現人力不足?<br />
##目前的國家考試,是否能檢測出應考人未來進入臨床就業之門檻能力?<br />
##目前是「臨床需求」引導考試與教學?或教學引導「考試」?<br />
##有關「基礎醫學」科目之命題及審題之建議:<br />
###「基礎醫學」科目之命題及審題工作,宜邀請臨床教師參與。<br />
###命題方向以臨床實務為導向? 試題要佔多少百分比?<br />

二、分組討論的結論:<br />
在引言後,由馬國興教授主持。馬教授邀請成功大學解剖學郭教授、高醫大生化學候教授、北醫大病理學朱教授、與中醫大藥理學陳教授,分享各學科教學及國考命題的經驗。隨後針對上列之各項困境與問題進行討論。經過與會師長與學生們的熱烈且廣泛的討論後,對於基礎學科在國考命題面臨的各項困境之結論為:<br />
#「教、考、用」三者間的配合是否緊密?<br />
##各校現行課程多以整合方式推動,但國考內涵未能配合。臨床、國考與教學須要進一步的整合。<br />
##記憶題考題數目應降低,取而代之則可提升情境題的比例。唯情境題應以自然且低爭議性為要。<br />
##許多老師希望考選部可分析並釋出各應試子科目之成績等資訊,作為各校教學參考。<br />
#有關「基礎醫學」科目之命題及審題之建議:<br />
##「基礎醫學」科目之命題及審題工作,宜邀請臨床教師參與。<br />
##建議可增加發放命題老師credit之對象;除了「出題」老師應給予credit外,對於關心出題趨勢提供分析數據、或者參與出題研習之師長,亦應給予適當的credit。<br />
##未來是否參考國際題庫措施,或採全英文命題?例如香港大學與香港中文大學的做法。進而達到醫師考試國際化認證,謹將此建議提供給考選部參考。<br />
#命題方向以臨床實務為導向? 試題佔多少百分比較恰當?<br />
##多數老師認為在國考命題,基礎學科與臨床的結合是共識。唯仍有部分基礎學科老師反應,考題難以結合臨床概念的原因,包括:在考選部出題嚴謹規定與歷年學生申訴風氣之下,難有靈活出題的空間。<br />
##純基礎題目與臨床相關題目的最適當比例,尚未有共識。唯依據新制醫學系工作小組之國考命題與能力規劃小組針對101年至103年進行分析結果,尚屬合理。<br />
#經討論後,多數老師認為在國考命題,基礎學科與臨床的結合是共識。各校對於國考命題,基礎與臨床的結合,其建議做法有二:<br />
##在審題階段,引入臨床教師做跨科審題。唯此作法或有曠日廢時,以及權責不明的風險。<br />
##在命題階段,由主命題之基礎教師,結合臨床教師作共同命題(但僅作為選項之一,而非強制、全面性的措施)。<br />
